Instructions

Please complete this form with the following attachments:

• Attachment an official bank statement or verification of deposit within the past six months. The amount certified must be equal or more than the estimated amounts for one academic year.
• Attach a copy of the biographical page of your passport

How to calculate the minimum balance of a bank statement:

• Freshman, Transfer students: Total estimated cost (tuition & fees, living expenses, books, supplies, insurance) – scholarship(s) = minimum balance stated on the bank statement

If you are currently studying in the U.S. on an F-1 visa, you must include the documents listed below:

• A copy of your current Form I-20
• Saint Mary’s I-20 Transfer Form

If you plan to bring dependents, you must include documents listed below:

• Passport of each dependent
• Proof of relationship to student (copy of birth certificate or copy of marriage certificate)
• Additional $4000 per dependent to total amount
